About Insurance

We are currently providers with the following insurance companies:

– Aetna

– CDS

-Friday Health Plan

– Healthscope/PEBS

– Hometown Health

-Medicare

-Prominence

-Senior Care Plus

-University of Utah

Your co-pay will vary according to your insurance plan and is required at the time of service. We also will provide you with a superbill that you can use to bill your own insurance if we are not a provider for your plan. Your therapist can help you answer some questions about your plan, but it is your responsibility to ensure coverage.

The following questions are good ones to ask about your insurance coverage:

  • Do I have a deductible and has it been met?
  • How many sessions per year does my plan allow?
  • Does my plan pay for out-of-network providers?
  • What is the copay per session?

Cancellation Policy

Your appointment time is reserved just for you. If you cannot come to your appointment, we can often schedule someone else who needs to get in on an urgent basis. Therefore, if you cancel an appointment within 24 hours you will not be charged. However, if you cancel with less than 24 hours notice you will be charged the full fee for the session. Illness and emergencies are dealt with on an individual basis with your therapist.

Good Faith Estimate

In January 2022, the federal government mandated the No Surprises Act that give clients who are not using health insurance the right to receive in writing a “good faith estimate” for your total out of pocket costs . We will provide this estimate upon scheduling and again once we meet you, and will renew it annually. You are never obligated to pay for any costs listed in your estimate before any service is rendered. You also have the right to dispute your actual and accrued costs, should they significantly exceed your good faith estimate. Please don’t hesitate to relay your questions or concerns about this to us.
